- Check or money order for the total premium.
- A recent color photo & description of any item(s) in the collection exceeding $2,000 in value.
Please do not submit: paper (including baseball cards), statuary, wine collections, records/cassettes/videos/CDs, furniture, fine art, china/flatware/stemware, stamps/coins or jewelry.
