Download the appropriate application (ie, stock, modified, antique/custom motorcycle, or collectibles) and any supplemental forms required by your state. To expedite the application process, please make sure to fill out the application and any supplemental forms entirely, leaving no blanks. If you prefer, applications may be submitted online via our Agents’ Site.
Please include the following when mailing an application:
- Completed application & any supplemental state forms.
- Check or money order for the total premium. Minimum premium is $30. We accept the applicant’s personal check or your agency’s check. Please contact us if you would like to pay by Visa or Mastercard.
- Recent clear, color photo of each vehicle. If vehicle the value exceeds $25,000, or is modified, please send four photos (two exterior; one interior and one engine).
- If there are any drivers in the household with less than ten (10) years driving experience, please complete the Youthful Driver Exclusion Form.
Please do not submit: Off-road vehicles, jeeps (unless they are 100% military) or vehicles used for racing or timed speed events. We are unable to process requests for "liability only." Liability is not available as a single coverage.
To complete your online submission: Select the state where your client's vehicle is registered. Download and print any supplemental forms for the appropriate vehicle type (e.g., Modified Vehicle). The Uninsured/Underinsured Motorist form is located on the back of the application (unless otherwise noted). DO put the binder number somewhere on all forms. DO NOT fill out the application, as this information has already been captured.
